Medical matters and official complaints cannot be dealt with via this form. If you have a query regarding a medical matter please telephone reception to make an appointment to see the appropriate person.

How to make a complaint?

We are always happy to improve our services to patients and the way we deliver them. If you have any comments or suggestions we would be pleased to receive them. 

Should you have cause to complain we have a proper complaints procedure and would be pleased to assist you in making use of this.  You can view a copy of our leaflet explaining this here; these are also available throughout the waiting area and from reception. 

You can also download the complaints form here. When completing the form please give as much detail as possible, this enables the Practice to investigate your complaint fully. This can be emailed using the address at the bottom of the form. 

Alternatively please ask to see the Practice Director, Christine Buckley if you have cause to complain.
