Some services provided by the practice are not covered by the NHS. This means that patients requesting these services may need to pay a private fee.

GPs are not obliged under the NHS contract to complete non-NHS forms or provide private medical reports. These requests are processed outside normal NHS work, and a fee will apply.

How to Request a Private Service

If you require a private form or medical examination, please:

1. Submit the form to reception or via the practice email.

2. Reception will advise you of the fee and expected timescale.

3. Payment will be required before processing begins.
