Should you have cause to complain we have a proper complaints procedure and would be pleased to assist you in making use of this. You can view a copy of our leaflet explaining this here; these are also available throughout the waiting area and from reception.
You can also download the complaints form here. When completing the form please give as much detail as possible, this enables the Practice to investigate your complaint fully. This can be emailed using the address at the bottom of the form.
